Insurance Terminology

Assignment of benefits:
An agreement between doctor and patient where the patient transfers to the doctor the right to receive future insurance payments that are owed the patient by an insurance carrier.

Capitation:
A set dollar amount that a third party payer pays to a doctor regardless of the number of services provided.

Carrier:
A third party that carries (or assumes) certain risks for a policyholder.

Claim:
A request for payment of a loss by a policyholder that may or may not come under the terms of an insurance contract.

Co-pay:
An amount paid by the insured for losses covered by a policy after the deductible amount has been met.

Deductible:
An out-of-pocket expense that a policyholder pays directly to the doctor before insurance cover any treatment costs.

Deposition:
A statement made under oath to obtain evidence in a legal matter.

Diagnosis:
A clinical description of a patient’s condition using terms that are accepted by the chiropractic profession and most other health care providers.

Disability:
The partial or total loss of mental or physical abilities caused by an injury or disease that prevents an insured from engaging in some or all of the duties of his or her usual occupation.

Exacerbation:
An increase in the severity of a condition(s) or the patient’s symptoms.

Fee for service:
The traditional method of payment for health care services where payment is made by the patient for specific services delivered by a doctor.

Gatekeeper:
An individual, often a medical doctor, who controls patient access to healthcare services for members of a specific group. Referrals from this “gatekeeper” are necessary to see a specialist.

HMO:
Health Maintenance Organization. Generally, a prepaid plan (not insurance) that offers certain health care services for a fixed monthly fee.

Impairment:
A loss, alteration or abnormality of psychological, physiological or anatomical structure or function that does not take into account the activities and job functions of an individual. (See disability.)

IME:
Independent Medical Examination. An examination arranged by a third party payer that is theoretically designed to “impartially” evaluate a patient’s disability or another doctor’s diagnosis or treatment plan.

Insurance:
An agreement by which one party (the insurer) assumes the risk of the payment of health care treatment faced by another party in return for a premium payment.

Lien:
A creditor’s claim against assets to secure a debt.

Managed Care:
A program that imposes controls on the utilization of health care services or the providers who offer such care.

Maximum Medical Improvement:
A point at which the patient’s care has reached his or her pre-incident condition, often ending the insurance carrier’s obligations.

Medical Necessity:
Health care services and supplies provided by chiropractors (DCs) that are appropriate for the evaluation and treatment of a disease, condition, illness, or injury.

No fault:
Generally a form of insurance in which a person’s losses from an automobile accident are paid by his or her own insurer, regardless of who was at fault.

Out-of-network:
A provision for reimbursement of services by a provider who is not a member of the patient’s managed care organization that usually involves a higher co-pay or a reduction in reimbursement.

Persoal Injury Protection (PIP):
A type of coverage in an automobile policy that pays for medical costs in case of an accident. Also known as Medical Payments coverage or “Med Pay.”

PPO:
Preferred Provider Organization. A network of doctors and hospitals that contract with an insurance company or employer to provide employees with services at competitive rates.

Personal Injury:
Usually associated with injuries sustained from an automobile accident, slip or fall incident, or harm caused by the negligence of others.

Pre-authorization:
The prior approval required by some third party payers before benefit payments are granted.

Provider:
Those persons who provide health care services, such as hospitals, physicians, chiropractors, nurse practitioners and others.

Reimbursement:
The payment of the expenses incurred after an accident or sickness, up to any limit specified in the policy.

Third-party payer:
Any payer other than the patient, for health care services, such as an insurance company, HMO, PPO or the government.

Treatment Plan:
A practitioner’s plan for treating a patient, over a period of time, for a specific condition.

Uninsured Motorist Coverage:
An insurance provision that pays for bodily injury to the insured, a family member or others in the insured’s care when the injury is caused by an uninsured, underinsured or hit-and-run driver.

Unusual, customary and reasonable fee:
A term that describes the amount that the doctor will charge for a particular service in a geographical area.

Wellness/Maintenance care:
Health care that is not prompted by sickness or injury but by an attempt to achieve or promote an optimum state of physical, mental and social well-being.

Worker’s Compensation:
A type of insurance that covers employee illnesses, injuries and disabilities occurring in the course of their employment.

What are the Education Requirements to Become a Chiropractor?

What are the education requirements to become a chiropractor?

Generally, a Doctor of Chiropractic Degree (DC) requires a minimum of eight years of study. The student is required to complete a four-year undergraduate degree which includes two years of pre-professional studies emphasizing chemistry, physics, biology, etc.

Upon acceptance to an accredited Chiropractic College or University, the student completes studies emphasizing anatomy, physiology, biochemistry, pediatrics, dermatology, genetics and other subjects related to health and disease. During the final year of Chiropractic school, the emphasis is on the treatment of neuro-musculo-skeletal problems, spinal misalignments or subluxations, biomechanical problems, differential diagnosis and x-ray diagnosis.

A one-year clinical internship must be completed prior to graduation in order to attain the Doctor of Chiropractic degree. After graduation, the Doctor must pass four demanding national board examinations and state examinations in the states in which they choose to practice.

FAQ: What Levels of Care Can I Choose From?

Depending on your condition, health care goals (feel better, get better, or stay better), finances and schedule, you may choose from different levels of care.

Acute/Relief Care – This initial treatment is intended to decrease inflammation, reduce spasms, and alleviate pain. The number of treatments necessary to accomplish this depends on your age, underlying spinal condition, condition duration and other lifestyle choices.

Corrective/Rehabilitative Care – During this level of care, we are primarily concerned with the strengthening of the soft tissue around the joint. This structural approach is designed to help prevent a relapse back into acute/relief care, and to aid in “holding” the correction in place and possibly creating structural improvements.

Maintenance or Wellness Care – Maintenance care prevents lapses of previous conditions and addresses new conditions before symptoms appear.

FAQ: What are the Effects of Subluxations on the Body?

Spinal subluxations alter normal nerve impulses and thereby cause pain and disease conditions, not only in the back, but elsewhere in the body. The nerve impulses travel along the nerves and exit the spine heading toward all of the internal organs to help control their function and different parts of the musculoskeletal system.

FAQ: Why Do Children Need Chiropractic Care?

Chiropractors treat the condition of life. Chiropractors check the spine for vertebral subluxations, nerve pressure caused by misaligned vertebrae, which cause interference in the flow of information from the brain to the various systems of the body. This nerve interference left uncorrected insidiously and negatively effects good health. Getting adjusted regularly helps each and every function of your body, promotion lasting good health.

Subluxations may occur at any time in our lives. For example, during the birthing process, difficult vaginal or obstetrical intervention (forceps, vacuum extraction or cesarean), can cause subluxations. Such procedures often disrupt the integrity of the delicate infants’ spinal column, altering the function of the nerves emanating from the upper neck area. This can lead to upper neck problems and increased susceptibility to ear, nose and throat infection.

Many times children have accidents and parents may believe that they are fine because there is no external evidence of damage. Spinal subluxations sometimes don’t signal an obvious symptom. The earlier children’s spines are checked, the earlier problems can be detected, preventing health problems from occurring and becoming serious. We recommend all children be checked regularly for nerve interference, even without symptoms present. Prevention is the best medicine.

If you know a child who is not under chiropractic care and suffers from any of the following problems, please urge the child’s parent to consider Chiropractic. Remember, vertebral subluxations lead to pressure on spinal nerves which leads to dysfunction.

Dr. Johnson has a gift and has helped my husband and me with acute back pain and will squeeze us in to see us at a moments notice. In addition, we have gone on and off for maintenance and never feel pressure to come in more than necessary. He is gentle, personable, kind and very talented. The best chiropractor I've ever been to. Dr Johnson knows where to work on you to help you heal and recover. He's patient, knowledgeable, and compassionate. Dr. Johnson and his team are great! Whether it’s just a quick tune up or addressing an injury there’s nowhere else I’d rather go. Even my wife and kids see him when needed. Visits are quick and to the point without sacrificing bedside manner. They’re also super military friendly!

While in Orange Country on business (from Eastern NC) I developed some pretty significant lower back pain while out on a run- the type of pain that made moving, or sitting, terrible. The idea of sitting in an airplane seat from one side of the country to another was akin to medieval torture. So I hobbled into Doc Johnson's office, was seen promptly. He helped me work out my lower back issue by the end of the week to where sitting in an airplane was bearable. I was across the country from home and needed some help, and he really came through for me. I really appreciate it!
